Error of the plurality of questions is quite common and is caused by the desire of the researcher to remove as much information in one question that leads to a logical violation, and therefore wrong to put the question.
A large group of typical errors in the questionnaires constitute a substitution of concepts.
Substitution of notions often takes place in issues when the sociologist tries to combine in a single concept two others, similar in content and form a new concept. The question: "Tell me, please, what, in Your opinion, should be changed in the medical and health institutions in the city?" the sociologist tried to unite the two concepts are close in meaning, "medical" and "Wellness" companies - in one General concept "Agency that monitors the health of the person".
